monsters hit extra

first off, great work guys, I really enjoy this game! I have noticed every once and a wile I will be attacked by a monster and it gets damage on me before it rolls to hit me, then it gets its normal attacks. has any one also noted this?

Are you sure you don't have a poison on you at the time?

Higher AC than you and gets in a random Critical Hit?
