Can someone please tell me where to go and who to talk to to start the silver scale quest?

Go towards prim. There will be a passage to the east of a map tile. Before you go into the cave go east you should see small steps(There's two map tiles there)

Still can't find it - which cave specifically are you referring to - the one with the two statues on either side of the entrance?

When you leave Stoutford castel go north heading to prim and bwm you see away on the mountain to go east with stairs. I forget the exact place without going back there.

Completing this is driving me insane! Some clues in reddit suggested it's just the beach to the west, I can't get to the shore. Is there a way north from the clearing to the river?

You can't go directly north from the clearing to the river. You have to go the long way round by exiting BWM area and circling up past Crossglen

Can someone answer my question who finished the "surprise" Quest with Halvor already?

How to satisfy him? So far i could always satisfy his needs after he moved (he's now near Remgard - long Bones).
Does it mean i've to send him away first in order to finish this task?
