Exactly!

As the translator, you know when you can use the original name (maybe bloodletter works great in Italian, just like the English often use the German Morgenstern instead of morning star for that weapon), when to go literal, or when to be creative to convey a concept, a feeling.

When you translate, you should do what you feel makes the game better in your language, even if it slightly alters the original meaning.

What to do when a joke is a word play that is untranslatable in your language? Drop the joke and keep the main meaning? Alter a bit the meaning to make a somewhat equivalent joke in your language? Make a literal translation and add another sentence with a local joke?

There is no correct answer. You have to choose what's best according to you.
