I'm level 18, what quests or areas should I be operating in? I'm at a point where things seem absolutely too hard or way too easy.

What quests have you done already?

Most recently I completed the wasp quest from the lumberjacks and I've cleared the undead prison.

You can try the Loneford quests or Stoutford quests.

I did the Loneford one where I found the disease source. Are there other quests I missed there? Stoutford quests have been a bit tough, maybe i need to grind around Stoutford some then.

You could head west from the prison, and try Blackwater Mountain.
