How to Not Miss?
It seems like no matter how much I raise my hit percentage, I still miss the vast majority of my attacks. How do I fix this? Am I playing the game wrong?

The important thing at first is to increase the AD at each level and save the skill point to always get IF when possible. Using the attack and escape strategy also helps. There are several forum posts that help you evolve faster and choose skills and equipment wisely

There is a graph done by Oskar that shows how AC works. I don't have time to find it in the forum's history. Maybe someone can find it

Re: How to Not Miss?
I'm sorry, I took a long time to answer this post and when I was typing I got confused thinking that you were complaining about AD.
As far as AC is concerned, I only raise it to level up after having at least AD 30 so I can attack any monster with high DR, I think the highest DR is 21.
The best alternative to improve AC in my opinion is through equipment.
You might consider taking at least 1 WA level, because it is a requirement for another skill that will be useful in more difficult areas.
I don't want to take more than WA 3, because it's 12 AC every 4 levels, while 4 level up can give 20 AC

Re: How to Not Miss?
Pay attention also to your attack cost. High PA mates make fewer attacks per turn.
The more attacks per turn you have, the greater the chance that you will hit at least 1 time.
For example, if your effective AC against a monster is 50%, with 2 attacks you have a 75% chance to hit at least 1. With the same amount of EAC and 3 attacks per turn, you hit at least 1 attack. 87.5% of the time
