Extraordinary items

Right now, I am looking for Black Defender and Shadowfang out of all extraordinary items available in game.
Legendary is beyond my time and energy for now.
I've gone through the main topic already but still have the following doubts.
1) Is it possible to have any (be it extraordinary or legendary) items purely by chance or the game keeps a track of number of kills and then decides to drop the items in any part of the area with the monsters set to drop them ?!?
I am asking it because the main topic emphasis on number of kills, but I did manage to get Shadowfang in one of my previous playthroughs, when I first started to head towards remgard while killing monsters randomly, and somewhere around midway but before crossing the irdeghs, I picked up the weapon as a loot drop.
Unfortunately, I immediately sold it and since then never got the chance to see in any of my subsequent playthroughs.
2) Not interested in spending points on magic finder skills. Are there any other tips to improve the chances of getting them?
3) Are measure items a sure shot indication of a monster dropping it soon?
For ex: I already have 20 jinxed rings. No clue about polished rings but it could be around 40 drops. Still no shadowfang. Though, I'll continue to try. Do I also need to have 200 polished rings?
