I found the bloodstained rope, headed back to Prim, and talked to every NPC, but nothing happened. Is there another step to trigger the NPC who tells me not to report it? (I didn't see them.)

You need to go back where you found the rope and walk around a little bit more

Also, I have an empty bottle from the mushroom cave, but the game doesn't seem to recognise it at the underground spring.

You've got to get the empty bottle from the potion maker in Fallhaven

Got it — thanks. The sequence of the quest might be a little off, but I'll see what happens when I come back with the right bottle.

Perhaps the empty bottles from the mushroom cave could be labelled as "soup bottles" and the quest could ask for an empty water bottle, to avoid confusion.

OK, I'm really at a loss. I've probably covered every square in the underground area at least twice (confirmed with the online map that I haven't missed any areas), I have the bloody rope, 10 large bottles of water and 10 flasks, the quest tab is telling me to report to someone in the village, but nothing happens when I exit and go back to Prim.

Could you post your quest log of 'climbing is forbidden'? That might give us an idea what's missing.

Ah, OK, that was it. From the quest description, I expected the person to be in my way as soon as I came down the ladder. Thanks!
