Damage and Attack chance

Example: AD 20-30, AC: 200

Is there any correlation between attack chance and the damage output?
I mean, (1) the higher the attack chance, the maximum is the damage number rolled by the dice on every successful hit on a target?
OR,
(2) A higher attack chance only improves the hits made on a target with fewer misses on every level up?

If (1) is also true then are there other ways to improve the chances of maximum damage?
Say, to achieve 25+ damage (out of 20-30 AD) on maximum hits per round!

What indicators (be it items or skills) determine the probability of maximum damage per hit?

It's been a while since I found it in the forums but there is a section that explained the formula a little bit, x % AC equaled y chance to hit and same for block and crit. It was a nice table too. It is the second one though, AC only modifies your percent chance to hit the target, with diminishing returns based on how much over the targets block chance. So there is a optimum range but everything you add still helps improve your chance. After a certain point though I think it is most helpful to just do AD. The others wouldn't be noticable differances.
