Yes, you are not the first one to see this. But none of the dev team ever sees it (except when moving at high speed in debug mode, but that doesn't count). I assume you do not see this very often?

Yeah. Even in debug mode I see it very rarely. It's a timing issue, and you and the monster have to move at almost exactly the same time for it to happen. As such, it's on the list, but not a particularly high priority. It's rare, and it doesn't break anyone's games.
